As a result of the Paperwork Burden Reduction Act, Saluda Nursing Center will not automatically furnish Forms 1095-C (Employer-Provided Insurance Offer and Coverage) for the 2025 tax year.
Right to Request a Copy
You have the right to receive a copy of your Form 1095-C upon request. To request a copy of your Form 1095-C or if you have any questions regarding this matter, please contact Human Resources.
How to Request a Copy
To request a copy of your Form 1095-C or if you have any questions regarding this matter, please contact our Human Resources department. If you are a resident of California, the District of Columbia, New Jersey, or Rhode Island, we will continue to automatically furnish you with a copy of your Form 1095-C.
